SS-31 (elamipretide) is a synthetic aromatic-cationic tetrapeptide that concentrates >1000-fold in the inner mitochondrial membrane, where it binds selectively to cardiolipin. This interaction stabilises cristae structure, optimises electron-transport-chain efficiency and reduces reactive-oxygen generation.
SS-31 is described as the first-in-class cardiolipin-protective compound, restoring mitochondrial bioenergetics in aged and diseased tissues across cardiac, renal, neural and skeletal-muscle models 123. It has progressed to late-stage clinical trials in mitochondrial disease.
