Cagrilintide is a lipidated, long-acting analogue of amylin, the 37-residue pancreatic hormone co-secreted with insulin. It activates amylin and calcitonin receptors, a mechanism distinct from and complementary to the GLP-1 pathway.
In a 26-week phase 2 dose-finding trial, cagrilintide 4.5 mg produced a mean weight change of −10.8% versus −3.0% with placebo and −9.0% with liraglutide 3.0 mg 1. A phase 1b study of cagrilintide co-administered with semaglutide 2.4 mg showed mean weight changes up to −17.1% at 20 weeks 2, the basis of the CagriSema development programme.
