Tirzepatide is a 39-amino-acid synthetic peptide based on the native GIP sequence, modified to activate both the glucose-dependent insulinotropic polypeptide (GIP) receptor and the GLP-1 receptor. A C20 fatty-diacid moiety provides albumin binding and once-weekly pharmacokinetics.
In SURMOUNT-1 (n = 2,539; 72 weeks), the mean body-weight change was −15.0%, −19.5% and −20.9% with 5, 10 and 15 mg respectively, versus −3.1% with placebo 1. Its dual-agonist pharmacology makes it a key comparator in research on incretin receptor cross-talk.
